Small correction to your table: the S-LiTE exercise prescription was two supervised group sessions and two individual sessions weekly, not two sessions in total. The distinction matters because "add some exercise" is not what was tested.

Having read your piece on what that device measures, I am no longer sure what I have been reassured about.

Nor are we. A handheld device measures impedance across the upper body and infers the rest, and the inference is least reliable exactly where you sit: older, substantial weight change, changing hydration. That is not a criticism of your consultant’s judgement, which may be sound on other grounds, but the device is not the evidence for it.

Your piece treats the one-quarter rule as discredited and then quotes fractions of one third and two fifths from the substudies as though those were more solid. They are group means from a hundred and forty people. Physician, heal thyself.

A fair hit, and we have amended the paragraph to carry the same caveat in both places. The distinction we should have drawn is that the substudy figures are at least attached to a stated population and a stated instrument, which the textbook rule is not. Neither is a constant.

It is achievable on an orbital trap with internal calibration and is tight for a quadrupole time-of-flight on external calibration. The row is drawn from a real certificate issued by a laboratory running the former. We should have said so, and the note now does.

You put every documentary finding to the company before publication, which is admirable and also means you are letting companies that do not reply escape coverage entirely. Silence should have a cost.

It does, and we should make it more visible. Non-response is recorded in the dossier register and published there. What we will not do is publish an inference we cannot support merely because nobody objected to it, and that constraint does protect the unresponsive. We accept the trade knowingly.

The composite test table you print as a model is unrealistic. No research supplier is going to run headspace GC and ion chromatography on every lot at these price points, and publishing an aspirational document as a benchmark just makes real certificates look worse than they are.

Twenty-four months is a common default and “industry standard” is an accurate description of the practice rather than a justification of the claim. The distinction we would press is between an expiry date, which asserts shelf life, and a retest date, which asserts only a review interval. The second is defensible without stability data. The first is not.

A fair criticism of the reporting. Parcel 7 was submitted for purity determination on arrival and returned a figure within a percentage point of the supplier’s stated value, which is consistent with the solid-state argument and proves very little on its own, since we had no pre-shipment measurement on that vial. The design fault is ours: a shipment study without a paired baseline sample cannot answer the question we most wanted answered, and the next round will.

Your rotation advice says site does not affect absorption in this class, and then says to rotate anyway. If absorption is unaffected, why bother?

Because rotation protects tissue rather than controlling absorption. Repeated injection into one small area produces lipohypertrophy, and absorption from lipohypertrophic tissue is blunted and erratic for any injected depot. Rotation prevents the condition that would make site matter. The advice is consistent; we should have made the causal order clearer.

Collection services are not interested in what was in the syringe. A pharmacy or local authority sharps point will take a rigid sealed container without inquiry, and the barrier you describe is built entirely of anticipated judgement. We would rather say that plainly than add to the lecturing.

A quibble on your dead-space figure. You give two to seven microlitres for insulin-type needles, which is right for fixed-needle syringes, but the detachable pen needle plus syringe hub combinations sold in this market are considerably worse and you should say so.

Accepted and amended. The figure we gave applies to integrated fixed-needle insulin syringes; detachable arrangements on a luer fitting can retain an order of magnitude more, which at small injection volumes is a substantial loss. The table now distinguishes them.

On the section about diode-array detection and peak purity, I would add that true peak purity assessment requires library matching or at least spectral comparison across the peak width. A homogeneous spectrum tells you the peak is probably pure. A spectrum that shifts across the peak tells you it is not, and that information closes a gap the article identifies correctly.

The table showing what each method can detect is valuable but incomplete. You show no row for C-terminal truncation or N-terminal truncation as distinct phenomena. These are not rare, and they often elute differently depending on which end is missing. A generic gradient might resolve them; an improperly designed orthogonal method might not. The capability matrix should separate these cases.

Your article argues for two orthogonal methods and reports the lower figure, but the people running a single twelve-minute method have a cost story you do not address. A full orthogonal pair doubles the turnaround and at least doubles the cost, which is why the market does not do it. The criticism of method disclosure is fair. The criticism that a single method is wrong is unfair to the constraints people operate under.

We are careful to say that a second method costs instrument time on a sample already in the autosampler, which is substantially less than twice the turnaround, but you are right that we underweight the commercial reality that a buyer setting a budget for testing is trading thoroughness for speed and price. Where we would push back is that those constraints are not technical or regulatory ones. They are market ones, and markets can change if enough buyers demand it.

Sub-two-micron columns changed everything about what is practical for peptide separations, but I would push back on the statement that particle size gains are costless. The pressure limit of most commercial instruments is three hundred bar, and trying to force 1.7-micron particles at eight millilitres per minute on a 4.6-millimetre column will send you there quickly. Peak capacity is not free.

Correct on the pressure cost, and that belongs in the method section. The trade-off is real, which is why many laboratories use core-shell superficially porous particles as a compromise: they are substantially cheaper than sub-two-micron packings, deliver most of the efficiency gain at a lower pressure, and the efficiency-cost-pressure triangle is the actual business decision people make. We should have named it.

On amber glass: it is not merely cheap, it is standard in the wider chemical supply trade for anything with a chromophore. The fact that this market ships peptides in clear glass is a sign of who is doing the filling more than of any decision about photostability.

On the basis of the synthesis and analytical literature, where epimer formation during solid-phase assembly and during storage at extremes of pH is well characterised. What is missing is not evidence that it occurs but evidence about how much of it is present in any particular commercial vial, which is a different absence and the one we should have named.

I have shipped temperature-sensitive material commercially for eleven years and your coolant arithmetic is right but generous. You assume the pack starts fully frozen. In practice packs are pulled from a freezer that is opened forty times a day, and a pack that starts at minus four with a soft core has lost a fair share of its budget before the box is closed.

The latent heat calculation assumes a fully solid pack at its melting point, and a partially thawed pack is exactly as much worse as the missing solid fraction. As a community pharmacist I would add one thing to your section on missed doses: the label window matters less than whether the patient then double-doses to "catch up". I have seen that twice this year and both times the patient believed they were following instructions.

What we can say is that the defensible range is narrower than the disagreement suggests, that the denominator matters more than the ratio, and that a clinician or dietitian can convert a range into a number for your body in a way that a magazine cannot.

My mother is eighty-one and on a low dose for her diabetes. Her weight is down nine kilograms and she now struggles to get out of a low chair, which she did not eighteen months ago. Nobody has measured anything. I do not know whether this is the drug, the weight loss, or being eighty-one, and neither does anybody I have asked.

That is the situation the missing endpoint produces, and we are sorry to have no better answer. A chair-stand time takes thirty seconds to measure and would at least establish a baseline against which the next six months could be judged. It is worth asking for by name.

The soft-tissue artefact point in your bone section is underplayed. In a patient losing twenty per cent of body mass the change in overlying tissue is well outside the range the calibration was validated over, and the published analyses do not report a sensitivity analysis for it. That is not a caveat, it is a gap.

You bought our cheapest tier, published the number it produced alongside a competitor’s most thorough package, and called the result a spread. It is not a spread. It is three different products, priced accordingly, and your own table says so two columns to the right of the headline figure.

This is the objection we thought was most likely and we think it is partly right. The comparison is of standard products at standard prices, which is what buyers actually purchase, and we said so. But the presentation invites the reading you object to, and the figure caption now states the tier alongside each result rather than leaving it to the method columns.
