The half-life to steady-state arithmetic deserves to be spelled out every time, because it is the single most useful calculation in this subject. Four to five half-lives to approach steady state means a weekly molecule with a week-long half-life takes over a month, and every dose-response impression formed before then is formed on incomplete exposure.

What is missing from the evidence base is any comparison of holding strategies. We have trials of schedules and no trials of the discretion applied to them, so every statement about how long to hold is extrapolation. Saying so plainly would be an improvement on most of what is written.

Tolerability-led escalation is the practice your article describes most sympathetically and it is worth naming what it gives up. Holding a dose because of symptoms is reasonable; holding it indefinitely because a schedule was never revisited is drift, and the two look identical from the outside.

A small thing. You give the semaglutide diabetes ladder as ending at 2.0 mg and the weight ladder at 2.4 mg, without explaining why the same molecule has two ceilings for two indications. It looks arbitrary and I suspect it is not.

It is not arbitrary — the two maxima come from separate dose-selection programmes with different primary endpoints, and 2.0 mg was established against 1.0 mg in a dedicated glycaemic comparison. We have added a clause. The underlying point, that indication shapes the ladder as much as the molecule does, is worth more space than we gave it.

Custody protects the supplier at least as much as the buyer. A result produced from a sample of unknown history is not evidence against anybody, and suppliers who understand that ought to be the loudest advocates for the practice.

That is the defect in one sentence. The badge is accurate and the impression it creates is not, and the gap is created by the display rather than by the determination.

Nobody expires anything. A badge granted three years ago is still on the page, still linking to a report about a lot that ran out long ago, and there is no mechanism anywhere in the chain that removes it. Decay is the missing feature.

The mechanism transfers; the incidence figures transfer only to the extent that your actual exposure matches the trial exposure, which is unknown unless the content has been measured. That is not evasion. It is the reason we argue for peptide content as a standard reported field rather than purity alone.

A reference interval is a description of a population, not a definition of health, and your piece is one of the few places I have seen that said so plainly. Ninety-five per cent coverage means one person in twenty who is entirely well will fall outside it on any given analyte, before anything at all has happened to them.

A mobile phase carrying trifluoroacetic acid suppresses ionisation substantially, which is why a laboratory running an excellent chromatographic method sometimes has to change it entirely before the same sample will give a usable spectrum. The two techniques want different solvents.

In defence of the retention-time comparison your article is rude about: run against a properly characterised standard on a stated method, co-elution plus a matching mass is genuinely strong evidence. The weakness is not the logic, it is that the standard is usually a previous lot of the same product.
